I am a manager that needs to complete a Leavers Form.

If you are a manager with an employee leaving SLaM, please ensure you complete an online Leavers Form.  It is linked to here, and can also be found on all SLaM desktops. The form is processed by Payroll and may take up to 1 month to process.  Please send Leavers Form queries to [e-mail]. 

Payroll enquiries can also be directed to - Tel: [phone]. Open Monday to Friday 8am to 5pm. Or via www.sbs.nhs.uk where you can register on their Self Service Portal.

If you have already completed a Leavers Form please allow time for this to be updated, and then for the details to filter through to LEAP.
